Lab and Diagnostic Billing That Starts With a Clean Requisition
Your lab sees a specimen, not a patient. Every missing member ID, diagnosis code or ordering NPI on the requisition becomes your denial. CureBytes catches those gaps before the claim goes out, so the tests you run get paid.

Lab Bill
Labs Bill on Thin Margins, and 2027 Could Make Them Thinner
Labs run huge volumes at low prices per test, which leaves little room for error. Medicare may be about to squeeze harder. Under PAMA, Clinical Laboratory Fee Schedule cuts of up to 15% a year could resume in 2027 and run through 2029. The Consolidated Appropriations Act, 2026 delayed them again, but only for one more year. Labs reported private payer data from the first half of 2025 by July 31, 2026, and CMS will use it to reset CLFS rates starting in 2027. Weak commercial rates can drag your Medicare rates down with them. Every underpaid commercial claim now costs you twice. Then there's the work itself. Your lab didn't take the history or pick the diagnosis. You rely on a requisition someone else filled out. A missing diagnosis code, an old insurance card or an ordering provider who isn't enrolled in Medicare turns a valid test into a denial with your name on it.
Provider
Where Labs and Diagnostic Providers Lose Revenue
Incomplete requisitions.
Missing ICD-10 codes, member IDs and ordering NPIs are among the most common reasons lab claims stall. Chasing the ordering office for them eats days you don’t get back.
Medical necessity and frequency edits.
Medicare’s national lab NCDs and local LCDs tie tests to specific diagnoses and frequency limits. Without a signed ABN when coverage is in doubt, you can’t bill the patient when Medicare says no.
Molecular and genetic tests.
In MolDX states, Medicare requires a DEX Z-code on many molecular claims, and commercial plans often want prior authorization through a lab benefit manager. Miss either one and a high-dollar test pays nothing.
Date of service rules.
For specimens collected during a hospital stay or outpatient visit, Medicare’s date of service rules decide whether the hospital or your lab bills the test. Get it wrong and the claim denies or gets recouped later.
Low-dollar claims nobody follows up.
When a test pays $20, staff skip the appeal. Across thousands of claims a month, those write-offs turn into real money.
How CureBytes Supports Labs and Diagnostic Providers
Requisition and Demographic Review
Every requisition is checked for diagnosis codes, insurance details and ordering NPI before billing, and missing data is requested from the ordering office right away.
Medical Necessity and ABN Checks
Tests are checked against NCDs, LCDs and frequency limits, with ABNs and GA or GZ modifiers applied where they belong.
Molecular and Genetic Test Billing
DEX Z-codes, prior authorizations and lab benefit manager requirements are handled before high-dollar claims go out.
CPT, PLA and Modifier Accuracy
Panels, PLA codes, QW, modifiers 90 and 91, and 26/TC splits for diagnostic imaging applied correctly on every claim.
Payer and Contract Monitoring
Payments are compared to your contracted rates and the CLFS, and underpayments get appealed, which also keeps your commercial rate data strong.
Denial Management and A/R Recovery
Every denial gets worked, low-dollar claims included, and aging balances are followed up on a set schedule.
Reporting by Test, Client and Payer
Collections and denials broken out by test, ordering client and payer, so you know exactly which tests pay.

Frequently Asked Questions
Everything You Need to Know Before You Decide
What types of labs and diagnostic providers do you work with?
Independent clinical labs, molecular and genetic labs, toxicology labs, pathology groups and diagnostic imaging centers. There’s no minimum volume.
Can you handle MolDX and Z-code requirements?
Yes. We attach DEX Z-codes where MolDX requires them and manage prior authorizations with commercial plans and lab benefit managers.
What happens when a requisition is missing information?
We contact the ordering office for what’s missing and hold the claim until it’s complete, so it doesn’t go out just to be denied.
How should we prepare for the 2027 CLFS changes?
Strong commercial payments help. We check every commercial payment against your contract, appeal short pays, and show you which tests are most exposed to rate changes.
What does it cost?
CureBytes is paid a percentage of what we collect for you. There’s no setup fee and no minimum retainer, so the cost moves with your test volume.
Is our patient data secure with an outside billing partner?
Yes. CureBytes maintains HIPAA and SOC 2 compliance across every account, whatever the size of your lab.
