Telehealth Billing

Telehealth Is Here to Stay. The Billing Rules Keep Moving.

Telehealth made up 5.51% of medical claim lines in Q1 2026, up 10.1% from the quarter before, according to FAIR Health. More than half of patients with a telehealth claim, 52.1%, had a mental health condition. Virtual care isn't a pandemic stopgap anymore. It's a core line of business. The rules haven't caught up. Medicare's telehealth flexibilities lapsed during the fall 2025 government shutdown. Congress then extended them through December 31, 2027 in the Consolidated Appropriations Act, 2026. The DEA's tele-prescribing flexibilities run only through December 31, 2026. Any practice planning a year ahead is planning around deadlines. Then there's the claim itself. Medicare pays POS 10 visits at the non-facility rate and POS 02 at the lower facility rate. It wants modifier 93 on audio-only visits, and it won't pay the newer telemedicine codes 98000–98015. Many commercial and Medicaid plans want modifier 95 or GT instead. Same visit, different claim, depending on who pays.

Provider

Where Telehealth Providers Lose Revenue

The wrong place of service or modifier.

POS 02 when the patient was at home, a missing 93 on an audio-only visit, or a 95 sent to a payer that doesn’t use it. Each one means a denial or a short payment.

Licensing and enrollment in every patient's state.

You need a license where the patient is sitting, and Medicare requires a separate enrollment in each state where you deliver care. One missing enrollment denies every claim from that state.

Payers that each play by their own rules.

Some states require payment parity with in-person visits. Many don’t. Commercial plans keep their own covered code lists, and out-of-state Blue members route through BlueCard to their home plan.

Deadlines that change what's billable.

When federal flexibilities expire or get extended, claims for affected services may need holding, rebilling or a different approach. Miss the switch and you either lose revenue or bill services that aren’t covered.

Remote monitoring left unbilled.

CMS added new RPM and RTM codes for 2026, including short-duration codes like 99445 and 99470. Without tracking device days and management minutes, those services go out unbilled or get denied.

How CureBytes Supports Telehealth Providers

Payer-Specific Claim Rules

POS 02 or 10, modifiers 93, 95 or GT, and the right E/M or telemedicine codes applied for each payer and each plan.

Multi-State Credentialing and Enrollment

State licenses tracked, Medicare enrollment filed in every state you serve, and commercial and Medicaid plans credentialed state by state.

Eligibility for Patients Anywhere

Coverage verified before each virtual visit, including telehealth benefits, copays and out-of-state plan rules.

Remote Patient Monitoring and RTM Billing

Device days, management minutes and patient consent tracked, so RPM and RTM claims meet each code’s requirements.

Regulatory Tracking

Medicare, DEA and state telehealth changes monitored closely, with billing rules updated before each deadline arrives.

Denial Management and A/R Recovery

Every denial worked back to its cause. Aging balances are followed up on a set schedule, payer by payer.

Reporting by State, Payer and Service

Visits, collections and denials broken out by state, payer and service type, so you can see where virtual care pays best.
